The CP Register is a confidential database of information about people with cerebral palsy in Australia.
Why is the CP Register needed?
The CP register is needed to:
- find out the causes of cerebral palsy
- find ways to prevent and manage cerebral palsy
- help in planning services for children and adults who have cerebral palsy.
You can participate in a number of ways:
- Visit the CP Register website, become a member and submit the required details

PDF) and consent forms (PDF), complete them and mail to CP Register PO Box 184 Brookvale NSW 2100. - Contact us and register directly by phone 02 9479 7200.
- Give permission for your health practitioner or education professional to:
a) submit your details on the CP Register website, or
b) send your name and contact details to the Register (CP Register PO Box 184 Brookvale NSW 2100). We will then contact you and obtain the required details.
