The CP Register is a confidential database of information about people with cerebral palsy in Australia.

Why is the CP Register needed?

  1. Visit the CP Register website, become a member and submit the required details

  2. Download the registration (PDF iconPDF) and consent forms (PDF iconPDF), complete them and mail to CP Register PO Box 184 Brookvale NSW 2100.
  3. Contact us and register directly by phone 02 9479 7200.
  4. Give permission for your health practitioner or education professional to:
    a) submit your details on the CP Register website, or
    b) send your name and contact details to the Register (CP Register PO Box 184 Brookvale NSW 2100). We will then contact you and obtain the required details.
